If you’re experiencing mental health and/or substance use issues Support House programs are client directed with the aim of improving wellness.
Our Adult Team responds to the needs of adults who experience mental health and/or substance use issues in Halton. We offer support coordination, housing, peer support, harm reduction, navigation, and distress/crisis support.
Our supports include a Justice Team that specializes in supporting the unique needs of individuals that have mental health and/or substance use issues and are justice-involved. Looking for our Peer Support programs?
Our Youth Team responds to the unique needs of youth in Halton aged 16 – 24. Early intervention and rapid response are crucial for effective short and long-term outcomes with youth. We offer support coordination, housing, peer support, harm reduction, navigation, distress/crisis support, and support moving into adult services as needed.
Just Be You is a youth-led peer program that provides social recreation, peer-support and service connections for ages 15 – 25 who are experiencing mental health, substance and/or addictions issues. It is funded by Shine Out Shout Out.
In North Halton, the Just Be You program is run in partnership with ROCK, ADAPT and the town of Halton Hills and operates out of the Youth Wellness Centre in Acton.
In South Halton, the Just Be You program is run in collaboration with ROCK and operates out of the Oak Park Neighbourhood Centre.
TAY Service Coordination is multi-agency committee that problem solves and assists youth who may benefit from an enhanced service plan and to successfully bridge youth into adult services.
