Community Engagement Survey

As an ongoing commitment to improving the quality of services you receive, seven mental health and addictions agencies (MH/A Alliance) across Halton have come together to work in partnership with you to co-design programs, services and supports that better reflect direct input from individuals like yourself within our community.

           


The Engagement Survey

This survey is designed to better understand how you would like to be included as a participant within this Virtual Lived/Living Experience and Family/Caregiver Engagement community.  By completing this survey you are giving Support House implied consent to follow up with you for future engagement initiatives.  Support House will be the custodian of the information you share through engaging in co-design, and Support House will only share your insights with the seven community partners identified above for quality improvement purposes.  You can opt out of participating or receiving engagement opportunities at any time by contacting:  [email protected]
