Together with the Connected Care Halton Ontario Health Team (CCHOHT), Halton Physicians Association, Oakpark/Churchill Neighbourhood Centre, the Mental Health + Addictions Alliance, people with lived experience, family and caregivers, Support House was granted funding to develop Halton’s first primary care Community Health Centre Mobile Health Team
This Community Health Centre Mobile Health Team will build pathways within encampments, local shelters, the Churchill Neighbourhood Centre, Milton, and Halton Hills.
Referrals will emphasize a “no-wrong-door” approach and will be accepted by community partners, primary care providers, specialists, ERs, and other acute care services.
Thanks to the support of Alex Irish & Associates, the Burlington Ontario Health Team (BOHT), and CMHA-Halton, our team has been equipped with a mobile response van to better support people wherever they are.
