The Mobile Health Team at Support House provides primary care services and social support to people in the Halton region. The team offers low-barrier health care to people who are not currently connected to care. Clients can access comprehensive care from the team through a variety of models that best suit their personal needs.

Who works on the CHC -Mobile Health Team?

Mobile Health Team services are provided by one of our staff, which could include a:

    • Nurse Practitioners and a doctor
    • Registered Nurse
    • Social Worker
    • Peer Support Worker

The Mobile Health Team also includes a Clinical Pathways Coordinator and Data Analyst and Program Manager to assist with non-client facing program work.

Who can receive service from the CHC – Mobile Health Team?

People who can receive service include clients with:

  • Healthcare Barriers: barriers to accessing primary care, such as no family doctor or nurse practitioner or an expired or lost health card.
  • Challenges with Social, Financial or Housing Needs: social related challenges, such as housing, income, trauma or discrimination or locating culturally appropriate services.
  • Unmet Health Needs: mental health or substance use health challenges, or multiple physical health concerns.

How does a person get service with the CHC – Mobile Health Team?

Community members can access Mobile Health Team services through a variety of routes and methods:

  • Drop-in Clinics and Medical Van – Located at a variety of community partner locations across Halton region, the Mobile Health Team offers walk-in services. No appointment or referral needed. See our drop-in clinic schedule HERE.
  • Virtual and/or Appointment-based Care – Virtual-based care is by appointment only. Staff can also schedule clients in for appointments at one of our clinic locations. For virtual and other appointment-based services a referral is required.
  • Outreach – Staff can connect with people on-site during outreach in Halton region. No appointment or referral is needed to address immediate needs. A referral may be needed if on-going care is required.

The Mobile Health Team also offers education and training sessions, group-based activities as well as specialty health clinics to community members.

Who can make a referral to the CHC?

The Mobile Health Team at Support House takes self-referrals as well as agency-based referrals for clients.

What types of services does the CHC – Mobile Health Team provide?

The Mobile Health Team offers a wide range of services to support client health and social needs. Services include (but are not limited to):

  • Health check-ups
  • Blood work, and tests
  • Vaccinations
  • Sexual Health testing
  • Wound care
  • Smoking cessation
  • Naloxone and safer drug use equipment
  • Addictions treatment
  • Counselling and peer support
  • Service and specialist referrals
  • Medical forms

Services are offered in a variety of formats from drop-in and appointment-based services to groups and outreach.
